Program Forms

Follow the steps below to register for this year's program.

1. Print, complete and return the Program Release Form for Carnegie Mellon. This form is required if your guest is attending a track or activity. The form MUST be received by April 18, 2008.

   Return your signed forms to:
   Dolores Heagy

2. Choose a track and activity that interests you.
   
   
   • Girl's Track Descriptions (for girls aged 9-15 years)
   • Boy's Track Descriptions (for boys aged 9-15 years)
     
     
3. Register for your favorite track and activity.
   
   
   • Girl's Registration
   • Boy's Registration
     
   
   
4. Print, complete and return the School Information Form to your teacher.
   
   
5. Print and review the Checklist to help you prepare for Take Our Daughters and Sons To Work® Day.
   
   
6. Volunteer to help with Take Our Daughters and Sons To Work®. Contact Gloria Gruber.
   
   
   • Help with Lunch from noon-1:30pm.
   • Become a Track Guide from 1:00-3:30pm.
